Assessments for children and young people usually draw on information from home and from school, because ADHD is assessed across settings rather than in a single room on a single day.
What tends to be useful
School reports, comments from teachers about attention and organisation, and a parent or carer’s account of early development are all helpful. So is anything that shows how the young person manages when things are unstructured.
What is not needed
You do not need a formal report from school before making an enquiry, and you do not need to gather everything in advance. PRC will tell you what is needed and how to share it securely.

Do not attach medical records or detailed history to a web form or a general email. Wait until you are told how to share documents securely.
