Booking appointments, describing their own difficulties and asking their own questions are skills. They are best practised before they are required.
What changes practically
Consent, confidentiality and how much a parent is involved all shift as a young person gets older. Those changes should be discussed openly rather than arriving unannounced.
Support continues
Needing support as an adult is not a failure of the support given as a child.
